This survey has five (5) parts.

At the end of each section, there's a 'next' button at the bottom of the page. On the final page, click submit to send your answers to us.

Please note that all information will be treated confidentially. Except for our webmaster and the survey administrator, nobody will see your name (if you enter) associated with your answers. Board members will only see aggregated results, and of course any comments that you may enter on a topic.

Goal is to assess the functioning of the website for survey-taking. The question content here is not approved. They are only examples based on a previous survey. I've tried to include various types of question formats: Check one-or-many boxes, check only one box (radio buttons), dropdown select, text area, text field. Please feel free to give false information. When testing is complete, I'll share the result pages with the team.
